Ohio Department of Health surveyors have determined procedural failures likely contributed to a COVID-19 outbreak at Newark Care and Rehabilitation nursing home.
75 residents and 49 staffers have tested positive since April 15. 28 residents have died. The state says the facility failed to perform adequate contact tracing to properly identify, quarantine, and appropriately test all residents with known exposure to a staffer who had tested positive in June.
