ODH Report Finds Fault With University Hospitals Over Embryo Storage Tank Malfunction

The Ohio Department of Health says University Hospitals in Cleveland made major mistakes before its fertility clinic storage tank malfunction in March.  

The department's report on the malfunction that killed four-thousand eggs and embryos found UH officials failed to inspect the storage tank, failed to heed alarms indicating a problem and improperly filled the tank with liquid nitrogen.  The hospital says it is working to correct those issues.
